How complex diabetic patients are assessed and managed in Tasmania.

Urgent/category 1	Phone the Consultant/Registrar to organise the urgent review. Fax the referral.	We aim to see these patients within four weeks, or sooner if clinically indicated.
Semi-urgent/category 2		We aim to see patients within 12 weeks
Routine/category 3		Next available appointment

Emergency referral process

A faxed referral is required for all new appointments.
Referrals are accepted from specialists, GPs and allied health professionals.
Indicate if a GP management plan and a GP team care arrangement is in place and include documents.
Clinic appointments are based on clinical priority and patients are allocated to the most appropriate clinic.
If you’re not the patient’s regular GP, that GP will receive correspondence for continuity of care.
