Continuing Care Teams

Depending on where you live in Tasmania, community mental health teams have different names. 

In Southern Tasmania, they are called Continuing Care Teams (CCTs).

In North and North-West Tasmania, they are called Adult Community Mental Health Services. 

This page refers to Continuing Care Teams (CCTs), but the teams work in a similar way. Different operating hours are listed under ‘Locations and opening hours.’

About Continuing Care Teams 

Continuing Care Teams (CCTs) are an integral part of community mental health services providing ongoing care to people with long-term mental ill-health and their families, friends and supports. 

CCTs generally see people aged between 18 years and 65 years. 

Location of care is usually determined by a person’s home address. 

CCTs work collaboratively with other aspects of mental health services, including inpatient, acute, and extended care services, child and adolescent mental health services and older persons mental health services. 

Our approach

CCTs provide specialist community mental health services that work towards personal recovery and wellbeing. 

We offer: 

• comprehensive assessments, 
• collaborative and person-centred treatment, 
• care coordination, and 
• advocacy. 

We listen to understand your perspective and care is tailored to individual strengths, personal recovery goals, choices, and needs. 

We recognise and value the role that families and friends play, and include them in care planning and provide support they might need. 

We also work with supports already in place or to strengthen existing community support networks. 

The length of care depends on each person’s choices, needs and requirements. 

How to access CCTs 

You can access services in several ways: 

If you have been discharged from a CCT within the last 90 days and require further support, contact the relevant team using the numbers below. 

If you have not accessed Statewide Mental Health Services before or it has been longer than 90 days, call:

People seeking support, families, friends, and support networks, and health professionals including your GP, can contact Access Mental Health to discuss referrals. 

People can also be referred to CCTs from other parts of Statewide Mental Health Services. 

Locations and opening hours

In Southern Tasmania:

In the south, CCT provide an extended hours service seven days a week, 8.30am-10pm.

In North and North-West Tasmania, the teams have a different name – Adult Community Mental Health Services, but work in a similar way:

Our teams

Many clinical and support staff make up our team, including: 

• consultant psychiatrists 
• other specialist medical staff 
• nurses
• clinical psychologists
• occupational therapists
• social workers
• peer workers (people with lived experience of mental ill-health, either for themselves or as a family member or support person)
• administration staff.
