Home and Community Care (HACC) Program

The HACC program assists people who need basic support services to continue to live in the community with dignity.

Clients include older and frail persons, and younger persons with disabilities who are at risk of inappropriate residential care if basic support services are not available.

HACC Services include:

  • Respite Care;
  • Home Nursing;
  • Personal Care;
  • Delivered Meals;
  • Home Help;
  • Home Maintenance;
  • In-home support (lifting, linen services);
  • Allied health services such as physiotherapy, occupational therapy and podiatry; and
  • Packages of care
