Glamorgan Spring Bay Community Nursing is located at Triabunna on the South East Coast of Tasmania. Community nursing covers the area from Bicheno in the north to Buckland in the south.


Registered Community Nurses attend people who require occasional or on-going support at home or in a nursing clinic to maintain their health and wellbeing.

Community Nurses provide the following:

  • Wound management
  • Continence advice and support
  • Palliative Care for people with a life-limiting illness
  • Pathology services
  • Medication management
  • Chronic Condition self management support
  • Vascular access - intravenous infusions and medications, PICCS and infusaports


Community Nursing Clinics - Spring Bay Community and Health Centre and Swansea Health Centre.

Clinics - Clients may book appointments for treatment at a time that fits with their daily schedule.

Referrals may be made by:

  • Individuals
  • Family
  • GPs
  • Carers
  • Health Care Providers

The person being referred must consent to the referral.

"Legs 4 Life"

This program provides

  • One on one information on how to care for dry, cracked or itchy skin, swelling in ankles and feet, slow to heal wounds or areas of scaring, aches and pains.
  • Assistance with choosing skin care products and stocking/socks.

Normal office hours are between 9:00am and 4:00pm.  After hours and when the office is unattended calls will be diverted to an answering machine.

All contact with the Community Nurses is through the Spring Bay Community and Health Centre Reception.

What to expect from Community Nursing

On admission to the Glamorgan Spring Bay Community Nursing Service, one of the registered nurses will conduct an initial assessment and plan care with the client and/or family.  During this first contact, the nurse will also deliver care if required.  This assessment may take longer than subsequent contacts and may be completed over a number of visits.


Clients will be discharges from the Glamorgan Spring Bay Community Nursing Service when:

  • Their goals are met
  • They no longer require our services
  • They are admitted to hospital or residential care
  • They move out of the area

Services may be limited or withdrawn if either the client's or staff's safety in compromised.
