About Us

We are community based professional nurses who work together with your usual doctor, hospital staff and other health care professionals to ensure that you receive consistent care of the highest quality.

What do we do?

After Hospital Care that is aimed at assisting you to return to your usual activities as soon as possible.

Community based nursing care that will assist you to maintain your independence within the community.

Community Nurses provide a range of care that includes assistance with:

  • Assessments and development of agreed plans of care
  • Catheter and continence management
  • Stomal therapy and support
  • Health education and promotion
  • Medication management including support and administration of medications via various routes and devices
  • Palliative care
  • Post-acute nursing care following discharge from hospital
  • Wound Care
  • Advice and support with short, long term and chronic medical conditions, such as diabetes, that may be affecting your ability to manage daily activities

Where possible appointments will be conducted at our community nursing clinic. This enables you to:

  • Book an appointment time so you can plan your day better: and
  • Receive your care in a relaxed, friendly environment with free parking and easy access to the clinic.

Accessing our Service

Referrals can be made by:

  • Hospital referral before discharge
  • General practitioner referral
  • Self referral
