HACC Program Manuals


The HACC Minimum Data Set collects a set of basic demographic information about HACC services. It also collects information on the number of units of services clients receive.

The HACC MDS is used to describe the program and what it does to improve planning and reporting and to evaluate the level of services being provided to clients.


HACC Program Discussion Paper (now closed)

The Department of Health has released a Discussion paper that describes how Tasmanian HACC Program policies and service delivery can better align with the future community care environment and provide better outcomes for clients, providers and government.

HACC Client Group Analysis

In 2014 - 2015, the Department of Health and Human Services engaged the University of Queensland to undertake research into the Tasmanian HACC Program client group.  This work used a variety of data to identify the profile of clients using the Tasmanian HACC Program.

Tasmanian Access Point - Final Report

The Australian Institute for Primary Care and Ageing (AIPCA) was engaged by the Department of Health and Human Services to conduct an evaluation of the Home and Community Care (HACC) Access Point Model in Tasmania
