If you have concerns for the safety or welfare of a child, call the Advice and Referral Line on 1800 000 123. We prefer all contacts to be made by phone so that we can make sure we have all the information we need to make an assessment.

If a child is at immediate risk and Police or medical assistance is required, dial 000.

For urgent matters, where a child or young person needs immediate protection, please call at any time.

Our general business hours are 8:30am and 5:00pm Monday to Friday, with after hours on-call services available for urgent calls.

This online form is provided as alternative method to report non-urgent concerns. Please note this form will require you to give your name and contact details. Your details will remain confidential at all times.

As an interim measure following the separation of the previous Department of Health and Human Services into two Departments, this notification form is provided on this website on behalf of the new Department of Communities Tasmania.
